Financial Assistance for Seniors Facing Health Challenges
Thrive & Purpose Foundation is committed to providing financial assistance and essential resources to individuals aged 50 and older in Kern County who are navigating chronic health challenges. While our current grant cycle focuses on supporting those managing heart failure, our long-term vision is to expand support to individuals facing a variety of health conditions. Eligibility & Application Process
✔ Eligibility: Applicants must be residents of Kern County, 50 years or older, and actively managing heart failure.
✔ Application Window: Applications are accepted year-round, with each individual eligible to apply once per calendar year.
✔ Submission & Review: Applications are carefully reviewed by the Thrive & Purpose Grant Committee. Please allow up to 90 days for review and approval.
How to Apply
Interested individuals can submit an application directly through the Thrive & Purpose Foundation website. Applicants will need to provide:
✔ Proof of age and residency (e.g., government-issued ID, utility bill).
✔ Medical cost details (e.g., copies of bills, receipts).
✔ A personal statement detailing your experience with heart failure and how this grant will help meet your healthcare needs.
